Targeted Case Management
Targeted Case Management (TCM) provides assistance to adults or children with serious mental illness or emotional disorders. TCM helps individuals and their families gain access to needed medical, social, educational and other services that are vital in the path to recovery. Eligibility for this service is based on current diagnosis, mental health treatment history, level of functioning and other medical criteria. Targeted Case Management includes Resource Coordination and Intensive Case Management.
Psychiatric Services are provided to individuals in need of medication management services. Psychiatrists and Advanced Registered Nurse Practitioners are available to evaluate medication needs and provide ongoing medication management for children and adults.
