Requesting Medical Records
IMPOWER has partnered with MediCopy, a trusted Medical Records Organization (MRO), to handle all medical records requests. MediCopy is fully HIPAA compliant and follows all state and federal regulations regarding the release of health information.
How to Request Your Records
For Active Patients, Parents, or Legal Guardians:
- Access via Patient Portal: Most records are available through your patient portal. Log in here.
- Need additional records? Submit a completed Authorization to Disclose Protected Health Information form:

For Third-Party Requestors:
Please submit:
- A written request for records
- A completed and signed Authorization to Disclose Protected Health Information form

What Information Is Needed?
To process your request, the authorization form must include:
- Client’s name and date of birth
- Recipient’s name, address, email, phone, and/or fax
- Type of records requested (e.g., evaluations, treatment plans, labs, medications)
- Signature and date from the client or legal guardian. If signed by a guardian, proof of guardianship or limited power of attorney is required
Note: Under Florida law, the minor client must provide written authorization for release of any substance use treatment records.
How Long Does It Take?
Please allow up to 30 days from the date your completed request is received to receive your records. Incomplete forms cannot be processed.
